If you would like to obtain a copy of your medical records, images or test results, we are here to help. To ensure privacy, we require that all request forms be signed and dated by the patient or the patient’s legally authorized representative, parent or guardian.

To obtain a copy of your medical records

Complete the above release of information form and follow one of these options:

1. Mail the form to:
   Health Information Department
   UF Health Flagler Hospital
   400 Health Park Blvd.
   St. Augustine, FL 32086
2. Fax the form to (904) 819-4915
3. Email the form to roi@flaglerhospital.org
4. Bring the form to the health information (medical records) department located on the 4^th floor, Suite #4005, open: Monday – Friday, 9 a.m. to 4 p.m.

To obtain radiology (x-ray) images/CD

1. Mail the form to:
   Radiology Department
   UF Health Flagler Hospital
   400 Health Park Blvd.
   St. Augustine, FL 32086
2. Fax the form to: (904) 819-4928
3. Bring the form to the radiology department located on the 2^nd floor lobby, open Monday – Friday, 7 a.m. to 4:30 p.m.

To obtain a copy of your bill or an itemized statement

Call our billing customer service department at 904-819-4539 or 855-438-2565. Our Patient Account Representatives are available to assist you Monday through Friday, 8 a.m. to 4:30 p.m.

Due to state and federal privacy laws, only the patient or the patient’s legal representative can obtain medical information. Please be prepared to show proof of such.
